Everything you need before your visit
Forms, insurance details, and office policies — all in one place so you are ready before you arrive.
New Patient Forms
Print and complete these before your first visit. Bring them with you, or fax them to the office.
Records Forms
Transferring care, or need a copy of your child's chart?
Sports Physical Forms (AIA)
Please print and bring all four with you to your appointment. AIA forms are updated each school year — make sure you have the current version.
ADHD / ADD Evaluation Forms
For any ADD/ADHD evaluation appointment, please have both completed ahead of time.
Insurance, billing & office policies
Insurance & Billing
We accept almost every health insurance plan and are contracted with most. Where we are in network, we bill your insurer directly. Self-pay discounts are available for uninsured patients or non-covered services — just ask. We accept all major credit cards, checks, and cash.
Co-Pays & Deductibles
Estimated co-payments, co-insurance, and deductibles are collected at check-in. A same-day well and sick visit, or a procedure needing follow-up care, may carry an additional co-pay or deductible from your insurer.
Newborns & Insurance
A parent's insurance card is accepted through baby's first-month visit. Your child must be added to a plan within 28 days after that, or claims may go unpaid.
Keeping Insurance Current
Please tell us about any changes to primary, secondary, or tertiary insurance — ideally at every visit, as soon as you are aware of them.
Ongoing Care Scheduling
Asthma and allergy follow-ups are needed every 3 months for medication refills. Dental clearances should be scheduled at least 2 weeks before any dental procedure.
Records & Fees
Your child's immunization record is free at their first visit ($5 to replace if lost). Records sent to another provider are free; personal copies are $1/page for the first 25 pages, $0.25/page after. A fee applies to provider paperwork such as FMLA forms.
